Add New Name Authority Term to Procedural Record

User Story

User may add a new name authority term to a procedural record via the predictive text field. If the user enters a term that does not exist in the name authority, a popup will ask if the user would like to add the term.

Save will add the term to the vocabulary, cancel should result in the term being removed from the field altogether.

The newly added term should appear in the "display name" field of the name authority record.

Note: For the 0.5 release, terms will automatically be added to the below-specified vocabulary of the Name authority; users will have a choice of vocabularies in 0.6/0.7.
